Survey: for the youth and family services Communities that Care Board

I am on the board of the communities that Care on Mercer Island.   The board is working to decrease drug and alcohol use by youth.   One area we are working on is creating messages that are effective, and delivered in the right medium.
Please, if you would take a coupld minutes to fill out the attached survey.   Just replace the number you are choosing with an X .  
Then if you could email it to me I will compile them with the others.  
We are interested in sixth graders through adults.
